Age UK East London is seeking a highly motivated and experienced Community Engagement Officer to join our Community Outreach team. As a key member of our team, you will play a vital role in supporting older adults with low-level support needs to live independently within the community.

Key Responsibilities:
  • Provide high-quality outreach support services to enable older adults to maintain their health and wellbeing.
  • Develop and maintain relationships with statutory and voluntary sector agencies to ensure access to appropriate services.
  • Work closely with service users to identify their needs and develop individual support plans.
  • Signpost people to relevant services and activities, promoting maximum choice and control.
  • Contribute to the development of shared working practices across the service network.
Requirements:
