Role Overview
We are seeking a dedicated and compassionate Support Practitioner to join our Mobile Homecare Team in Helensburgh. As a key member of our team, you will work closely with colleagues to provide high-quality care and support to vulnerable adults in their own homes.
Key Responsibilities
- Provide person-centred care and support to individuals in their own homes, promoting independence and well-being.
- Work collaboratively with colleagues to assess and meet the needs of service users, ensuring their safety and dignity.
- Develop and maintain positive relationships with service users, their families, and other stakeholders.
- Contribute to the development and implementation of care plans, ensuring they are tailored to individual needs.
- Participate in regular team meetings and contribute to the continuous improvement of our services.
Requirements
To be successful in this role, you will need:
- Experience of working with vulnerable adults in a care or support setting.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with service users and colleagues.
- Ability to work flexibly, including evenings and weekends, to meet the needs of service users.
- Knowledge of care and support principles, with a commitment to person-centred practice.
- Ability to work as part of a team, with a positive and flexible attitude.
