Job Title: Health Visitor

We are seeking a skilled and compassionate Health Visitor to join our team at Derbyshire Healthcare NHS Foundation Trust. As a Health Visitor, you will play a vital role in promoting the health and wellbeing of children and their families in our community.

About the Role

This is an exciting opportunity to work in a dynamic and supportive team, providing high-quality health visiting services to a defined corporate caseload. You will be responsible for delivering the Healthy Child Programme, working closely with families and other agencies to promote health and wellbeing.

Key Responsibilities
  • Provide evidence-based public health nursing services to a defined corporate caseload, supporting and enabling families to make informed choices about their healthcare.
  • Lead on the delivery of the Healthy Child Programme, identifying packages of care based on the Universal, Universal plus, and Universal Partnership plus models of service delivery.
  • Work collaboratively with other agencies and partners to ensure a seamless delivery of services.
  • Provide leadership and supervision to other members of the Public Health Team, ensuring effective delegation and work within Derbyshire Healthcare NHS Foundation Trust policies and procedures.
Person Specification

We are looking for a qualified and experienced Health Visitor who possesses excellent communication skills, both verbal and written. You will have a strong understanding of the 0- framework and be able to work in a sensitive and appropriate manner with children, young people, and their families/carers from diverse backgrounds.

You will be a skilled practitioner in breast and infant feeding, with regular training and clinical assessments as required. You will also have experience of working in a team, supervising, monitoring, mentoring, and assuming specific line management duties where applicable.
