Position Overview
An exceptional opportunity is available for a Registered Nurse to become a vital member of our Specialist Palliative Care team.
The Specialist Palliative Care team consists of Clinical Nurse Specialists and Specialist Physicians. The dedicated nurses collaborate across adult wards to assist patients with specialized palliative care requirements, focusing on symptom management and providing emotional support to both patients and their families.
As part of this specialized team, the Clinical Nurse Specialists will engage in clinical responsibilities, quality audits, educational initiatives, and contribute to the development of local policies and guidelines.
This role encompasses 30 hours per week, with a combination of permanent and fixed-term responsibilities.
The successful candidate will be expected to work standard hours, including weekends.

Key Responsibilities
As a member of our team, your responsibilities will include:
- Supporting all ward personnel in delivering palliative and end-of-life care
- Mentoring staff to ensure high standards of palliative and end-of-life care
- Managing a clinical caseload effectively
- Providing level 2 psychological support for patients and their families
- Participating in audits and, where applicable, research, and developing reports and action plans
- Delivering educational sessions to both post and pre-registration healthcare professionals regarding specialized palliative care
- Contributing to the formulation of policies, protocols, and guidelines related to palliative and end-of-life care as necessary
- Proactively engaging in service development initiatives
- Seizing opportunities for professional growth as an autonomous practitioner in a specialized nursing or educational capacity

Job Responsibilities
- Managing a designated caseload of patients, providing information and support to empower patients in making informed decisions about their care.
- Collaborating closely with medical and nursing staff, multidisciplinary teams, and departmental heads.
- Ensuring patients are seen within established timeframes and targets.
- Visiting inpatients as necessary to maintain continuity of care and provide advice/support as needed, ensuring thorough documentation of all clinical actions and communications.
- Accepting referrals from medical/nursing/MDT staff for patients meeting the referral criteria.
- Regularly reviewing caseloads and discharging patients appropriately in accordance with team guidelines.
- Effectively communicating sensitive information related to conditions with patients and caregivers in a timely and comprehensible manner.
- Liaising with other Specialists, Consultants, GPs, Allied Healthcare Professionals, and various agencies.
- Providing accurate written information following patient consultations to GPs and other team members as required.
- Ensuring timely completion of summary care records for discharged patients and maintaining relevant clinical information in IT systems.
- Functioning as an autonomous practitioner, responsible for managing and prioritizing workload based on patient and service needs.
- Assisting the Line Manager in providing clinical and managerial leadership and support to nursing and clerical staff.
- Deputizing for senior colleagues in their absence as necessary.

Qualifications

- Registered Nurse on the appropriate part of the NMC Register
- Completion of a Mentor Preparation Programme or demonstrable mentorship experience relevant to the clinical area
- Post-registration qualification and/or experience pertinent to the specialty
