We are seeking a highly skilled and compassionate Nursing Associate to join our innovative CAMHS Tier 4 service. As a key member of our multidisciplinary team, you will play a vital role in delivering intensive community support to young people and their families, promoting holistic and person-centered care.
Key Responsibilities- Work on a shared caseload with the team, supervised by a registered nurse, to provide comprehensive care and support to young people up to 18 years old.
- Contribute to nursing assessments, including initial assessments, nursing formulations, and nursing recommendations.
- Design and implement clinical interventions, including care planning, and monitor the physical and mental well-being of service users.
- Liaise with other professionals, including GPs, hospital departments, and social care, to ensure seamless care coordination.
- Deliver therapeutic interventions, under supervision, to meet the physical, emotional, and psychological needs of individuals and carers.
- Arrange patient and carer monitoring visits and attend as part of a planned package of care.
- Discuss and explain care plans with patients and families or carers, using Easy Read information.
- Write and review care plans under supervision from a registered nurse and update electronic care records.
- Undertake health education work, including the impact of mental health illness on overall wellbeing and support for individuals and families.
- Monitor patient medication concordance and administer medication in accordance with regulatory guidelines.
- Registered Nursing Associate on the NMC register.
- Completion of Nursing Associate foundation degree qualification.
- Ability to recognize own limitations in the context of the nursing associate role.
- Understanding of the importance of following procedures and treatment plans.
- Ability to support, supervise, assess, and act as a role model to nursing associate students, other learners, and healthcare support workers.
- Previous experience of working with young people or adults with a learning disability or Autism.
