Support Coordinator Role

We are seeking a highly skilled and compassionate individual to join our Stroke Recovery Service team in the Salford area.

This is an exciting opportunity to work with stroke survivors and their families, providing support and guidance to help them recover and regain their independence.

Key Responsibilities:

  • Provide emotional support and guidance to stroke survivors and their families
  • Develop and implement personalized support plans to meet the needs of each individual
  • Collaborate with healthcare professionals and other stakeholders to ensure comprehensive care
  • Conduct regular assessments and reviews to monitor progress and adjust support plans as needed

Requirements:

  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong organizational and time management skills
  • Knowledge of stroke recovery and rehabilitation principles
