About the Role

We are seeking a highly skilled and compassionate Support Service Coordinator to join our Care and Support Team at Thirteen Group. As a key member of our team, you will be responsible for providing support and guidance to vulnerable client groups, including young parents and their families.

Key Responsibilities
  • Develop and implement person-centred support plans to help individuals achieve their goals and improve their quality of life.
  • Work closely with individuals to assess their needs and provide tailored support to address their challenges.
  • Manage risk and safeguarding for all residents, ensuring their safety and well-being at all times.
  • Collaborate with other teams and agencies to provide a comprehensive support service to our customers.
  • Conduct outreach work in the community to engage with individuals and families, providing support and guidance as needed.
Requirements
  • Experience of working with vulnerable client groups, including young parents and their families.
  • Good understanding of adult and child safeguarding, including the complexities and challenges that may arise.
  • Ability to work independently and as part of a team, prioritising tasks and managing workload effectively.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals and teams.
  • Valid driving licence and access to a vehicle, with the ability to travel to various locations as required.
