About the Role

We are seeking a skilled Social Worker to join our multidisciplinary team at St Mary's in Warrington. As a Social Worker, you will play a vital role in providing care and support to individuals with complex needs, including Acquired Brain Injuries (ABI), progressive neurological conditions, Autism Spectrum Conditions (ASC), and Deaf Mental Health problems.

Key Responsibilities
  • Conduct comprehensive social care assessments and risk assessments to identify individual needs and develop personalized care plans.
  • Facilitate group activities and support service users to access community resources and services.
  • Participate in Interagency Care Review (ICR) meetings to ensure coordinated care and support.
  • Manage internal investigations and liaise with local authorities to ensure seamless care transitions.
  • Provide education and training to hospital staff on safeguarding matters and best practices.
Requirements
  • Registration with Social Work England.
  • Qualification in Social Work (degree level).
  • Experience working in a similar environment.
