The successful candidate will be accountable to the Primary Care Network and may provide services across multiple GP Practices within the PCN, depending on operational needs and support requirements. You will be joining a dedicated team in a supportive environment where your contributions will be genuinely appreciated, your expertise acknowledged, and your professional growth encouraged.

We have several positions available and are open to discussing flexible working arrangements. We are primarily looking for individuals who can commit to 30 hours per week, Monday to Friday, during standard operating hours.

Key Responsibilities

- Conduct assessments and triage patients, including same-day evaluations, and provide definitive treatment as appropriate, which may include prescribing medications in accordance with established policies, patient group directives, NICE guidelines, and local protocols, or referring to other members of the healthcare team as necessary.

- Utilize advanced clinical skills to educate patients, promote self-management, and empower them to make informed decisions regarding their healthcare.

- Perform and interpret electrocardiograms (ECGs).

- Collaborate as part of a multidisciplinary team within the PCN.

- Collect pathological specimens, including blood samples and swabs, within the scope of practice.

- Maintain precise health records using Systm1, ensuring accurate completion of all necessary documentation related to patient care and registration.

- Communicate effectively with patients and, when appropriate, their family members and caregivers, conveying complex and sensitive information regarding their health needs.

About Our Organization

Wentworth 1 Primary Care Network comprises seven member practices, providing primary care services to over 52,000 patients in the Rotherham area.

Our mission is to collaborate in coordinating, planning, and sharing resources to develop a robust and resilient primary care offering for all our patients.

This innovative approach brings together partners from various sectors, including health, social care, and the voluntary sector, to design and deliver services that better meet community needs, improve health outcomes, and utilize resources more efficiently. This includes the introduction of additional roles within our practices, such as Clinical Pharmacists, Social Prescribers, and Care Coordinators.

Job Duties

- Triage, assess, and treat patients in their homes or at the practice as directed. Make necessary referrals in collaboration with the referring or duty clinician to other members of the primary healthcare team, community services, or social care as appropriate.

- Assess, diagnose, plan, implement, and evaluate treatment and care for patients presenting with undifferentiated diagnoses on an acute basis, seeking guidance from the referring or duty clinician when necessary.

- Discuss with the referring or duty clinician the needs of patients with complex health conditions requiring specific treatment, hospital admission, or referral.

- Conduct clinical examinations and assessments from both physiological and psychological perspectives, interpret test results, and plan clinical care accordingly.

- Ensure that clinical practice is safe, effective, and within the boundaries of competence, acknowledging limitations.

- Advise patients on general health care and minor ailments, referring to other members of the primary and secondary healthcare teams as necessary.

- Operate as an autonomous practitioner within general practice or community settings, providing care for patients and families in accordance with the Health and Care Professions Council Standards and Proficiency, as well as Standards of Conduct, Performance, and Ethics. Maintain personal and professional clinical standards and development.

- Prescribe or issue medications as appropriate, following national policies, patient group directives, and local pathways, in accordance with personal competence.

- Share knowledge and skills with colleagues, promoting peer review and best practices within the work environment to ensure the delivery of a safe and effective service.

- Collect pathological specimens, including intravenous blood samples and swabs, and perform investigatory procedures as requested by GPs, only undertaking procedures for which you are appropriately trained.
