Cambridge City 4 Primary Care Network Ltd. is seeking a skilled Clinical Pharmacy Specialist to enhance and deliver pharmacy services within our network. The successful applicant will collaborate with a dedicated team of Clinical Pharmacists and Pharmacy Technicians to support and advance services across multiple GP practices, serving a diverse patient population.
Key Responsibilities
The Clinical Pharmacy Specialist will:
- Manage chronic disease care and conduct comprehensive medication assessments for patients with complex medication regimens.
- Act as a primary resource for general practice staff regarding medication-related inquiries.
- Facilitate the repeat prescription process, address urgent prescription requests, and ensure accurate medication reconciliation during transitions of care.
- Promote and implement strategies for safer prescribing practices.
- Provide expert clinical guidance on medication optimization and quality enhancement initiatives.
- Oversee certain elements of the quality and outcomes framework and enhanced service provisions.

Cambridge City 4 PCN Limited was founded to support local GP practices as part of a national initiative. Our network has expanded significantly, employing a multidisciplinary team of professionals dedicated to personalized care and rehabilitation services. Our leadership team consists of experienced Clinical Directors who are actively involved in our partner practices.
We foster a supportive and innovative workplace culture, encouraging professional growth and development. Our Clinical Pharmacists receive comprehensive support throughout their career progression, including pathways for independent prescribing qualifications.
Each Clinical Pharmacist is paired with a dedicated Pharmacy Technician, working collaboratively within designated practices. We emphasize teamwork and peer support through regular collaborative sessions, enhancing learning and professional relationships.
Qualifications
Essential:
- Master's degree in Pharmacy (MPharm) or equivalent.
- A minimum of two years of post-qualification experience.
- Strong understanding of General Practice operations.
- Exceptional communication and interpersonal skills.
- Ability to convey complex information clearly to diverse audiences.
- Proficient in managing and advising on medication optimization for long-term conditions.
- Strong IT skills and the ability to mentor colleagues.
- Self-motivated and capable of working under pressure.
Desirable:
- Advanced therapeutic knowledge and understanding of evidence-based healthcare principles.
- Experience in a GP surgery or Primary Care Network environment.
- Independent Prescriber status or intent to achieve it.
