We are seeking a highly skilled and compassionate Staff Nurse to join our Patient & Carer Wellbeing Service (PCWBS) team at The Myton Hospices. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality, patient-centred care to individuals living with life-limiting illnesses and their families.
Key Responsibilities- Coordinate the assessment, planning, and evaluation of individualized care plans, working closely with patients, families, and carers to ensure their needs are met.
- Provide compassionate and evidence-based care, maximizing the specialist palliative care and end-of-life skills of staff.
- Engage with patients and families to deliver individual and group activities of support and education, promoting holistic care and well-being.
- Work collaboratively with the PCWBS nursing team, volunteers, and other members of the multidisciplinary team to ensure seamless care delivery.
- Proactively communicate with other healthcare providers to ensure effective information sharing and resource utilization.
- Significant post-registration experience in palliative and end-of-life care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
- Teaching and assessing skills, with experience in needs assessment and care planning.
- Good knowledge of assessment and symptom management, with a commitment to evidence-based practice.
- Flexibility with working hours and the ability to travel within Coventry and Warwickshire.
