Role Overview:


As a Specialized Support Practitioner, your role will involve providing guidance and assistance to vulnerable tenants with complex needs, particularly those with a history of rough sleeping.

Your primary focus will be to build and maintain strong personal connections with tenants, enhancing engagement, overcoming barriers, and preventing tenancy breaches.

You will manage a caseload of up to ten clients, although this number may vary based on case complexity. These clients will come from various projects and services across the local area, making multi-agency collaboration essential.

Key Responsibilities:
Conduct needs assessments for at-risk and vulnerable tenants.
Provide and coordinate effective support, advice, and assistance to tenants to ensure they can maintain their tenancies.
Assist tenants in accessing support, care, and health services.
Maintain regular contact with tenants, implementing support plans and undertaking appropriate support actions.
Provide case studies and reports on outcomes within the service.

Qualifications / Requirements:
A valid UK driving licence and access to a personal vehicle, with business insurance for roles involving travel.
Experience working with vulnerable adults with complex needs and/or challenging behaviours.
Understanding of benefits and housing benefits systems.
