We are seeking a highly motivated and enthusiastic registered nurse to join our community nursing team at Epsom and St Helier University Hospitals NHS Trust. As a Band 6 Nurse, you will play a key role in delivering high-quality patient care and contributing to the development of our community nursing services.
Main Responsibilities- Assess and manage the care needs of patients in their own homes, community settings, and in-reach to hospitals.
- Work closely with multidisciplinary teams, including GPs, community matrons, and other healthcare professionals, to deliver comprehensive care.
- Develop and implement individualized care plans, taking into account the patient's physical, emotional, and social needs.
- Provide education and support to patients, families, and carers to promote self-management and independence.
- Contribute to the development and implementation of community nursing policies and procedures.
We are looking for a nurse with:
- Registration with the Nursing and Midwifery Council (NMC).
- A valid driving license and access to a vehicle for work purposes.
- Teaching qualification and experience of teaching and training others.
- Evidence of continued professional development post-registration.
- Experience of team management and multi-disciplinary working.
- Relevant post-registration experience in community nursing.
