Job Title: Lead Clinical Assessor / Senior Healthcare Professional

Key Responsibilities

As a Lead Clinical Assessor, your responsibilities will encompass evaluating individuals in accordance with the National Framework for NHS Continuing Healthcare and Funded Nursing Care. Your duties will include:

  1. Assessing new referrals and conducting patient visits for assessments and reviews.
  2. Collaborating with families and various stakeholders to ensure comprehensive care.
  3. Reviewing eligibility for NHS Continuing Healthcare and overseeing care planning to guarantee that all patient needs are met effectively.
  4. Monitoring and enhancing the quality of care provided, utilizing your expertise to improve patient experiences.
  5. Offering professional guidance to patients, caregivers, and multi-agency colleagues, including directing them to suitable services and resources.
