Main Responsibilities- Work collaboratively with the Rheumatology Consultant Pharmacist and Rheumatology Highly Specialist Pharmacist to develop and implement effective pharmacy services for the rheumatology specialty.
- Provide independent prescribing services for patients with rheumatological conditions, including autoimmune and inflammatory diseases.
- Manage ongoing pharmaceutical care for patients on drug treatment for rheumatological and related musculoskeletal conditions.
- Develop and maintain medicines management systems to ensure safe and effective use of medications.
- Provide pharmaceutical expertise to multidisciplinary teams and attend weekly multidisciplinary team meetings.
- Contribute to the development of protocols, policies, and guidelines for pharmaceutical practice in rheumatology and acute medicine.
- Provide clinical ward pharmacy services and support to the pharmacy patient services team.
- Deal with pharmaceutical queries and provide advice to medical and nursing staff.
- Clinically screen prescriptions to ensure safe and effective use of medications.
- Support antimicrobial stewardship initiatives and collect clinical and cost outcome data.
- Ensure accurate medication histories are obtained for patients and provide advice to patients and carers on medication adherence and self-management.
- Work closely with GPs, community pharmacists, and other healthcare professionals to answer outpatient prescribing and clinical queries.
- Record major interventions and investigate adverse drug reactions.
- Audit and risk manage selected areas of practice to promote continuous quality improvement.
- Mentor junior pharmacists and contribute to the appraisal process.
- Supervise medicines management technicians and other dedicated pharmacy staff.
- Contribute to the training of pharmacy undergraduates, medical students, and other healthcare professionals.
We are looking for a highly skilled and experienced Clinical Pharmacist with a strong background in rheumatology and acute medicine. The ideal candidate will have:
- Pharmacy degree and UK registration as a pharmacist.
- Previous hospital pharmacist experience and postgraduate certificate or equivalent in Clinical Pharmacy Practice or Rheumatology.
- Experience of audit, quality systems, medicines guideline or policy development.
- Proactive approach to applying specialist pharmaceutical knowledge to enhance patient care.
- Ability to manage time and prioritize effectively.
- Adaptable and flexible approach to work.
- Ability to work under own initiative and as part of a team.
- Computer literacy.
Desirable qualifications include:
- Qualified Pharmacist Independent Prescriber.
- Experience of teaching.
- Experience relevant to the specialist area.
- Evidence of Continuing Professional Development (CPD).
- Pharmacoeconomics experience.
