About the Role

We are seeking an experienced Registered General Nurse (RGN) to join our team as a Clinical Nurse Specialist. As a key member of our homecare team, you will play a vital role in delivering high-quality patient care and support to individuals in their own homes.

Key Responsibilities
  • Deliver clinical care and support to patients in their own homes, workplaces, or communities
  • Manage and coordinate patient care plans, ensuring seamless delivery of services
  • Collaborate with multidisciplinary teams, including healthcare professionals and support staff
  • Provide education and training to patients and their families on medication management and self-care
  • Conduct regular assessments and reviews to ensure patient needs are met
Requirements
  • Valid NMC PIN and registration as a Registered General Nurse (RGN)
  • Proven experience in clinical nursing, preferably in a homecare setting
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic IT skills and proficiency in using electronic patient records
