Job Title: Adult Nurse Manager

We are seeking a highly skilled and experienced Adult Nurse Manager to join our team at Leaders in Care. As a key member of our community care team, you will be responsible for providing face-to-face support to adult clients and their families, while creating personalized care plans for each individual.

Key Responsibilities:
  • Develop and implement effective care plans for adult clients, taking into account their unique needs and circumstances.
  • Provide training and support to carers, ensuring they have the necessary skills and knowledge to deliver high-quality care.
  • Work closely with the multidisciplinary team to ensure seamless care coordination and communication.
  • Respond to and investigate clinical complaints, in line with policy and risk management.
  • Submit clinical reports and updates to CCGs regarding their packages as per contractual agreements.
Requirements:
  • RGN with an active NMC PIN.
  • Supportive management style that can bring out the best in others.
  • Up to date experience of tracheostomy care and ventilators within the last 2 years.
  • Advanced communication skills verbally and written to liaise effectively with patients, families, and the MDT.
  • Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management.
  • Full UK Driving Licence essential.
