About the Role

We are seeking a highly motivated and experienced Physiotherapist to join our team as a Band 6 Community Specialist Physiotherapist. This is an exciting opportunity to work in a dynamic and supportive environment, contributing to the delivery of high-quality patient care in the community.

Key Responsibilities
  • Work closely with multidisciplinary teams to assess and develop care plans for patients in the community.
  • Provide specialist physiotherapy services to patients with a range of conditions, including respiratory and musculoskeletal disorders.
  • Develop and maintain strong relationships with community healthcare providers, including GPs and social care teams.
  • Contribute to the development and implementation of physiotherapy services in the community, including the design and delivery of education and training programs.
  • Work collaboratively with colleagues to ensure seamless transitions of care between hospital and community settings.
Requirements
  • Registered Physiotherapist with the Health and Care Professions Council (HCPC).
  • Postgraduate qualification in a relevant area of physiotherapy, such as respiratory or musculoskeletal physiotherapy.
  • Experience of working in a community setting, including experience of assessing and developing care plans for patients with complex needs.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible approach to working hours and locations.
