Position Overview

We invite qualified Nurses and Allied Health Professionals to consider a unique role within our Falls Prevention Service as a Geriatric Risk Assessment Specialist. This specialized team operates within the Primary, Community, and Dental services division, conducting thorough assessments for elderly patients both in clinical settings and at their residences throughout the region.

As a Geriatric Risk Assessment Specialist, you will engage with individuals who have experienced falls, evaluating their risk factors in accordance with established guidelines, and developing tailored strategies to mitigate these risks. While much of your work will be conducted independently, our team convenes weekly for collaborative support and professional development. Your assessments will include various clinical tests, such as ECGs and blood sampling procedures.

Key Responsibilities

The responsibilities of the Geriatric Risk Assessment Specialist include:

  • Preparing for patient evaluations by reviewing medical histories, current medications, and referral details.
  • Conducting Comprehensive Geriatric Assessments, which involve gathering detailed accounts of the patient's fall history, monitoring vital signs, and performing balance and cognitive tests, alongside phlebotomy and ECG procedures.
  • Compiling assessment reports to be shared with the patient's General Practitioner and the patient themselves, including recommendations for medication management and strategies to reduce fall risks.
  • Collaborating with team members on case discussions and meeting with our Consultant to review complex cases.
  • Facilitating necessary referrals based on assessment outcomes.
  • Utilizing electronic health record systems to access patient data and document final reports.

Job Requirements

The Geriatric Risk Assessment Specialist is responsible for evaluating patients who have fallen, identifying their risk factors, and creating a plan to address these concerns.

This role necessitates a comprehensive understanding of patient histories, referrals, and medications, as well as effective communication with patients and their families. The assessment process will involve various tests to determine the level of need or impairment.

The outcomes of these assessments will guide the Specialist in formulating strategies to manage fall risks effectively.

The Falls Prevention Service primarily operates during weekdays, with potential weekend shifts based on demand. Patient visits may occur on a bi-weekly or regular basis, depending on individual needs. The team aims to foster neighborhood collaboration while maintaining a countywide service, which may require travel beyond typical regions.

Strong communication skills are essential for this position, as multiple patient record systems are utilized.

Mentoring new team members and students is also a vital aspect of this role.

Required Knowledge and Qualifications

Essential

  • Understanding of geriatric care principles.
