About the Role

We are seeking a dedicated and compassionate Nurse Practitioner to join our multidisciplinary team at the Community Intermediate Care Centre. As a key member of our team, you will play a vital role in providing high-quality patient care and contributing to the effective working of the unit.

Main Responsibilities
  • Work collaboratively with a multi-disciplinary team to provide nursing and rehabilitation care for in-patient beds.
  • Take charge of the unit in the absence of senior colleagues, managing patient admissions, care planning, and discharge processes.
  • Contribute to the development and implementation of care plans, ensuring patient needs are met in a timely and effective manner.
  • Supervise less experienced staff and students on placement, providing guidance and support to ensure high-quality care delivery.

Requirements
  • Registered General Nurse with current professional registration.
  • Commitment to delivering high-quality care and supporting change management.
  • Excellent communication and interpersonal skills, with the ability to innovate, inspire, and promote patient safety.
  • Ability to identify patient needs and contribute to care planning, ensuring timely and effective interventions.
