We are a reputable organization in York, working in partnership with the NHS and York Council to provide high-quality care services. Our Reablement service has been running for several years, and we are now looking for caring and self-motivated individuals to join our team.

Job Role: Reablement Healthcare Worker

We are seeking individuals who have a passion for improving people's lives to support our clients in their own homes. As a Reablement Healthcare Worker, you will be working alongside our clients to help them manage their personal care, gain confidence, and achieve their goals.

Key Responsibilities:

  • Supporting individuals who have been discharged from hospital or have experienced a change in circumstances
  • Helping clients to manage their personal care and independence
  • Working closely with clients to set and achieve goals
  • Providing emotional support and companionship
