We are seeking a highly skilled and compassionate Nurse Specialist to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. Key Responsibilities- Conduct comprehensive clinical assessments and develop individualized care plans to promote patient independence and wellbeing.
- Collaborate with healthcare professionals to ensure seamless care coordination and effective communication.
- Provide education and support to patients and their families to promote mental health awareness and self-management.
- Participate in multidisciplinary team meetings to discuss patient progress and develop strategies for improvement.
