**About the Role:**

As a Support Worker, you'll be responsible for providing person-centered care and support to our service users, helping them to gain more independence in their daily lives. You'll work closely with our team to develop and implement personalized care plans, ensuring that each individual receives the support they need to achieve their goals.

**Key Responsibilities:**

* Provide high-quality care and support to service users, including personal care, meal preparation, and household tasks
* Assist service users with daily living activities, such as bathing, dressing, and using the toilet
* Support service users to access their local communities, including shopping, socializing, and engaging in leisure activities
* Develop and maintain positive relationships with service users, their families, and other stakeholders
* Work collaboratively with the care team to ensure that each service user receives a high-quality, person-centered service
