Key Responsibilities- Provide personal care and support to clients with complex needs, including assistance with daily living tasks, medication management, and mobility support.
- Develop and maintain positive relationships with clients, their families, and other stakeholders to ensure seamless care and support.
- Work collaboratively with our multidisciplinary team to identify and address client needs, develop care plans, and implement interventions.
- Conduct regular risk assessments and implement safety measures to minimize the risk of accidents and injuries.
- Maintain accurate and up-to-date records of client care and support, including progress notes, medication records, and incident reports.
- Participate in ongoing training and professional development to enhance your skills and knowledge in areas such as epilepsy, stroke care, and mental health.
- Experience working with clients with complex needs, including those with epilepsy, stroke, and mental health conditions.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and colleagues.
- Ability to work independently and as part of a team, with a flexible and adaptable approach to meet the changing needs of clients.
- Highly organized and able to prioritize tasks effectively, with a strong attention to detail and commitment to maintaining accurate records.
- Valid driver's license and access to a reliable vehicle, with a willingness to use your own car for work-related tasks.
