Position Overview
We are looking for a committed and enthusiastic Rehabilitation Support Practitioner to become part of our dedicated team at Midlands Partnership NHS Foundation Trust. In this role, you will play a crucial part in providing exceptional rehabilitation services to individuals facing various physical health challenges, including musculoskeletal disorders, neurological conditions, and post-operative recovery.
Your responsibilities will include collaborating with qualified therapists to deliver tailored rehabilitation programs and working independently within your professional scope. This position is dynamic and will evolve based on the needs of the service and feedback from both patients and staff. Your insights will be vital in shaping the future of this role.
This position will involve conducting scheduled visits to patients' residences.
Key Responsibilities
The role encompasses managing a diverse caseload of adults in close collaboration with the rehabilitation team. Many patients will present with complex health requirements. Within your professional capacity, you will have the opportunity to perform comprehensive assessments in the patients' home environments, utilizing manual assessment techniques and clinical reasoning skills.
The Rehabilitation Support Practitioner will assist various healthcare professionals to ensure that the physical, emotional, and psychological needs of individuals are met effectively.
- Implement and advance rehabilitation plans established by qualified therapists.
- Conduct follow-up assessments post-hospital discharge, including triage and both subjective and objective evaluations for suitable patients. Within your competency, identify the need for multidisciplinary team involvement and establish SMART goals for each patient.
- Facilitate the discharge of appropriate patients from the service, ensuring adherence to established pathways, completing necessary referrals, and providing timely discharge documentation while connecting patients to community resources.

Essential Qualifications
- NVQ Level III in Health and Social Care or equivalent.
- GCSE Grade C or above in English and Mathematics.
- Strong written and verbal communication skills.
Experience Requirements
- Significant recent experience as a Therapy Assistant within the NHS or private sector.
- Experience working with patients requiring nursing, therapy, or social care.
- Experience in a multidisciplinary team setting.
- Familiarity with care and treatment plans.
