Adults Social Worker Assessment Team

We are seeking a qualified Social Worker to join our Adults Service in Surrey. As a key member of our team, you will be responsible for conducting assessments of individuals' needs for social care and support, considering physical, emotional, and environmental factors.
Key Responsibilities:- Conduct thorough assessments of individuals' needs, identifying areas of support and developing tailored care plans to promote independence and well-being.
- Provide advice, guidance, and practical support to individuals and their families/carers regarding available services and resources.
- Liaise with healthcare professionals, community agencies, and other stakeholders to coordinate and deliver integrated care services.
- Safeguard vulnerable adults by identifying and responding to instances of abuse, neglect, or exploitation, following established protocols and procedures.
- Advocate for clients' rights and interests within the social care system, ensuring they receive appropriate support and access to necessary resources.
- Recognised Social Work qualification (Social Work Degree or Diploma in Social Work) and full Social Work England registration.
- Recent or significant relevant experience in a similar role, with a strong understanding of social care principles and practices.
