Job Title: Support Worker

Job Summary:

Voyage Care is seeking a dedicated and compassionate Support Worker to join our team in Bridgwater. As a Support Worker, you will play a vital role in enhancing the quality of life for individuals with complex needs, promoting their independence, and supporting them in achieving their goals.

Key Responsibilities:

  • Provide personal care and support to individuals with complex needs, including assistance with daily living activities, mobility, and communication.
  • Develop and implement personalized care plans, working closely with the individual, their family, and other healthcare professionals.
  • Encourage and support individuals to participate in activities that promote their physical and emotional well-being, such as exercise, socializing, and hobbies.
  • Maintain accurate and up-to-date records of individual's progress, including care plans, risk assessments, and incident reports.
  • Collaborate with other healthcare professionals, including nurses, doctors, and therapists, to ensure comprehensive care and support.

Requirements:

  • Previous experience in a care or support role, preferably in a residential setting.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and colleagues.
  • Ability to work flexibly, including evenings, weekends, and bank holidays.
  • Basic knowledge of health and safety procedures, including manual handling and first aid.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
