About the Role

Primary Integrated Community Services (PICS) is seeking a highly skilled Pharmacy Technician to join our team of Clinical Pharmacists in a Clinical Support Role. As a Pharmacy Technician - Medicines Optimisation Specialist, you will play a vital part in ensuring the safe and effective use of medicines within our Primary Care Network (PCN).

Key Responsibilities

  • Undertake patient-facing and patient-supporting roles to ensure effective medicines use, through shared decision-making conversations.
  • Carry out medicines optimisation tasks, including effective medicine administration, supporting medication reviews, and medicine reconciliation.
  • Support the Clinical Pharmacists in Structured Medication Review, including organising necessary monitoring tests prior to SMR.
  • Provide expertise to address both public health and social care needs of patients, including lifestyle advice, service information, and tackling local health inequalities.
  • Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients.
  • Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally.
  • Promote Electronic Repeat Dispensing to patients.
  • Promote online ordering through the NHS App.
  • Support practice reception teams in streamlining prescription requests, allowing GPs and Clinical Pharmacists to review more clinically complex requests.
  • Attend weekly Care Home Ward Rounds with GPs, face-to-face and virtually.
  • Support the implementation of national prescribing policies and guidance within GP Practices.
  • Support the PCN in delivering on QIPP agenda, QOF, IIF, and other contracts.
  • Support the PCN in reviewing and developing practice policies for CQC requirements.
  • Carry out housekeeping while in patient records, dose optimisation, syncing medicines, etc.
  • Conduct prescribing audits (as requested by Senior Clinical Pharmacist).
