Job Summary:
Piers Meadows Recruitment is seeking an experienced Senior Community Physiotherapist to join our team in Carlisle and district area. As a key member of our multidisciplinary team, you will play a vital role in providing rehabilitation and care to patients with a wide range of long and short-term conditions.
Main Responsibilities:
- Deliver specialist assessment and rehabilitation services to patients in their own homes, community hospitals, and residential care settings.
- Work closely with Commissioners, GPs, Acute Services, and third sector organizations to provide seamless care and support.
- Develop and implement individualized care plans to promote independence, quality of life, and prevent hospital admissions.
- Collaborate with multidisciplinary teams to provide rapid response care and support patients with frailty, long-term conditions, and those at risk of falls.
Requirements:
We are looking for a highly skilled and motivated Senior Community Physiotherapist with experience in community health and rehabilitation. You will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
