Candidate Profile

You will lead initiatives to support adults in living well at home. As part of a friendly and supportive team, your responsibilities will include managing a caseload, conducting Care Act Assessments, developing Support Plans, performing Mental Capacity Assessments, making Best Interest Decisions, and providing guidance to internal and external services to enhance well-being and independence.

Strong communication skills are essential, along with the ability to establish effective professional relationships with various disciplines and local health and social care services to facilitate a coordinated response for individuals and families.

If you are a newly qualified social worker, our ASYE Programme is available year-round for applications.

Roles are available in our Neighbourhood and Integrated Urgent Care Teams.
