About the RoleWe are seeking a highly skilled and experienced Registered Nurse to join our team as an Integrated Hospital Discharge Nurse - Clinical Leader. This is an exciting opportunity to work with a dynamic and forward-thinking organization, Livewell Southwest, that is committed to delivering high-quality care to patients in their own homes.
Key Responsibilities- Lead and manage the discharge team, ensuring the coordination and delivery of high-quality evidence-based discharge assessments and onward care planning.
- Assess, diagnose, implement, plan, and review complex needs and packages of care in partnership with patients, carers, and multi-professional agencies.
- Use clinical reasoning to formulate individualized programmes of care and provide intervention plans for people with a wide range of complex conditions.
- Develop and maintain effective working relationships with the multidisciplinary team, ensuring a cost-effective, quality service to patients and carers within a defined geographical area.
- Coordinate and manage a caseload or caseloads, delegating tasks within the skill mix of the team.
- Supervise, monitor, develop, and audit standards of care delivered to patients and carers by the multidisciplinary team.
- Enhance the skill mix of the multidisciplinary team and ensure that the ever-changing health needs of patients are met.
- Act as an advisor, professional leader in healthcare issues and clinical practice, which are often complex and challenging.
- Support links across and liaison with patients, carers, and the wider health, social care, and voluntary community.
- Triage and accept appropriate referrals directly from other disciplines or refer to other agencies where appropriate.
- Case manage, coordinate packages of care, and work within the framework of the Care Coordination Team by providing holistic care and demonstrating positive leadership skills.
- Ensure privacy, dignity, and human rights are respected for all patients and carers.
- Report and record all incidents and near misses relating to health, safety, security, fire, physical violence, aggression, and verbal abuse.
- Be responsible for ordering, monitoring, and authorizing suitable equipment within individual competence.
- Maintain accurate records, which are confidential, up-to-date, legible, and that all care given has to be documented.
- Registered Nurse with a minimum of 2 years' experience at Band 5 or above.
- Relevant clinical experience to degree level in a specialist area.
- Demonstrable leadership skills and ability to motivate and manage change.
- Good communication skills, influencing and negotiating skills.
- Ability to recognize the professional accountability and responsibility attributable to this post.
