The Single Point of Access service plays a crucial role in delivering timely advice, information, and assessment for referrals to adult care and community health services. We are dedicated to continuously improving our offerings to ensure outstanding outcomes for those we serve.
Role Overview
In this position, you will work alongside a multidisciplinary team of Health and Social Care professionals to evaluate and make informed decisions regarding complex cases. Your focus will be on empowering residents to make choices and maintain their independence. We prioritize your professional development, offering supervision and training opportunities to enhance your skills.
Please note that this role requires an enhanced Disclosure and Barring Service check.
