We are seeking a highly skilled and compassionate Liaison and Diversion Support Worker to join our team at Lancashire & South Cumbria NHS Foundation Trust.
The successful candidate will be responsible for providing high-quality support services to vulnerable individuals in contact with the Criminal Justice system, working closely with our Liaison and Diversion Practitioners to identify, assess, and refer individuals to appropriate health and social care services.
Key Responsibilities- Assess patient's individual care needs and agree and refer on to appropriate agencies.
- Involve the individual in the planning and prioritization of their activity where possible.
- Provide short-term intervention to encourage individuals to engage with services and attend their initial appointments.
- Complete and contribute to robust risk assessments and risk management plans.
- Deliver and promote positive risk-taking, where appropriate, to improve quality of life and maximize occupational performance/independence.
- Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
- Utilize agreed outcome measures to review the effectiveness of process, as part of Liaison and Diversion Service.
- Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
- Assist in ensuring that the aims and objectives of the service are fulfilled and to identify factors which may inhibit these from being achieved.
- To attend relevant clinical meetings / forums as requested.
- To plan and manage own delegated workload on a day-to-day basis.
- Education to Level 2 NVQ or equivalent experience.
- Ability to navigate systems.
- Knowledge of the local community and an understanding of the Criminal Justice system and identified vulnerabilities.
- Awareness of a range of vulnerabilities and its impact on individuals.
- Basic knowledge of signs and symptoms of a range of mental disorders and its effects on individuals.
- The ability to use electronic record-keeping systems.
- The ability and skill to form positive therapeutic relationships, whilst maintaining professional boundaries.
- Up-to-date knowledge of safeguarding.
- Experience working with individuals with a complex range of health and social care needs.
- Experience and/or sound knowledge of the Criminal Justice System.
- Experience of working in a community-based setting.
- Comply with health and safety.
- Commitment to attendance at work.
