As a key member of our community care team, you will be responsible for providing face-to-face support to adult clients and their families, while creating personalized care plans for each individual.

Key Responsibilities:

  • Design and deliver tailored care plans specific to an individual's needs
  • Effectively manage the multidisciplinary team through strong communication and teamwork
  • Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs
  • Deliver nursing interventions, including the management of medicine delivery
  • Build and maintain sophisticated relationships with clients' families
  • Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care

Requirements:

  • RGN with an active NMC PIN
  • Supportive management style that can bring out the best in others
  • Up-to-date experience of tracheostomy care and ventilators within the last 2 years
  • Desirable: ICU/ITU/CCU experience, advanced communication skills, and a full UK driving license
