Our aim is to improve care for patients by working collaboratively across primary care and our partners as part of the Integrated Care Partnership.
Job Summary
We are seeking a General Practitioner to join our team and support the delivery of our Extended Access Hubs service. As a key member of the team, you will work collaboratively with clinical and non-clinical colleagues to deal with a wide range of primary care presenting health care needs.
Main Responsibilities
- Foster and maintain strong links with all services across the PCN and neighbouring networks.
- Explore the potential for collaborative working and take opportunities to initiate and sustain such relationships.
- Demonstrate the use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organizations CCGs).
- Can recognize personal limitations and refer to more appropriate colleague(s) when necessary.
- Liaise with other stakeholders as needed for the collective benefit of patients.

Person Specification
- Experience of working within the NHS, with general public & team working.
- Well-developed IT skills and an enthusiasm for new applications and software.
- Good time management and highly organized.
- Ability to use initiative, judgement, resourcefulness, common sense and local knowledge.
- Interest in quality improvement work.
- Able to obtain and analyze complex technical information.
- Recognizes priorities when problem-solving and identifies deviations from the normal pattern and is able to refer to seniors when appropriate.
- Able to work under pressure and to meet deadlines.
- Produce timely and informative reports.
- Gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course action where there may be significant barriers.
- Work effectively independently and as a team member.
- Demonstrates accountability for delivering professional expertise and direct service provision.
Requirements
- Eligible to work in the UK.
- Able to carry out duties with or without premises adaptation.
- GMC registration.
