The successful candidate will be an experienced nurse who will provide high-quality care for patients in a general practice setting. They will work autonomously and collaboratively with other healthcare professionals to assess, diagnose, and treat patients with a range of medical conditions.
Key Responsibilities- Provide safe and effective care for patients, working within their professional boundaries.
- Conduct face-to-face consultations, home visits, and telephone consultations as part of daily surgeries.
- Work collaboratively with the multi-disciplinary team to meet the needs of patients and support the delivery of policy and procedures.
- Take a lead role in coordinating the clinical management of patients residing in local Nursing and Residential homes.
- Build effective working relationships with other healthcare professionals to deliver excellent primary healthcare services.
The ideal candidate will be an experienced nurse with a strong background in primary care. They will possess excellent clinical decision-making skills, be able to work autonomously, and have strong communication and interpersonal skills.
A strong understanding of policy and procedures, as well as the ability to work collaboratively with other healthcare professionals, is essential for this role.
