BACKGROUND

Healthcare Central London (HCL) operates as a GP Federation serving the Central London borough area. This organization provides support to 33 General Practices and 4 Primary Care Networks (PCNs). HCL manages various NHS contracts, including a Care Navigation Service, Clinical Pharmacy Service, Community Dermatology Service, and Partnership in Practice (PiP), which is an out-of-hospital service provided to its 33 General Practice members.

Our vision is to be acknowledged as a premier GP provider network, operated by clinicians for the benefit of our local community. We aim to achieve this by collaborating with patients and partners to ensure the sustainability and independence of general practice.

Our approach focuses on delivering exceptional support to our 33 General Practice members and managing our NHS contracts in a way that benefits our Practices and their patients by providing services that are economical, local, prompt, and familiar.

As we continue to grow in our 16th year, we are committed to representing General Practice within the planned Integrated Care System (ICS).

JOB ROLE

The Pharmacy Technician will assist the Clinical Pharmacy team in managing medication to ensure a safe, efficient, and consistently effective model of care within General Practice, including supporting a cluster of GP practices.

The primary objective of this role is to enhance safe and cost-effective prescribing practices to improve patient care quality. The individual will aid patients in optimizing their medication by implementing approved protocols, refining repeat prescribing processes in general practices, promoting repeat dispensing and online ordering, minimizing clinical risks, and striving to reduce medication waste.

Additionally, the post holder will foster a deeper understanding of medication optimization principles among practice teams and advocate for best practices in line with therapeutic advancements.

This role will also involve supporting the GP Federation in meeting national standards, implementing NICE guidelines, and utilizing medication optimization initiatives. We provide a collaborative environment where our Clinical Pharmacist Team works closely to enhance patient care and experience.

KEY TASKS AND RESPONSIBILITIES

• Assist the Clinical Pharmacist at the PCN level in delivering medication management within the general practice framework.

• Implement pharmacy-related components of the Network DES, focusing on medication optimization aspects of the Investment and Impact.

• Execute medication switches in GP practices following agreed protocols, reviewing patient records, updating records, and informing patients accordingly.

• Conduct housekeeping tasks within patient records, such as dose optimization and medication synchronization.

• Enhance prescribing processes within general practice.

• Promote electronic repeat dispensing and online ordering.

• Report on key performance indicators for CPGP to illustrate added value for patients and general practice in accordance with NICE guidance.

• Utilize and maintain relevant information systems and databases, including incident reporting, medication optimization, prescribing data, cost savings, and patient outcomes.

• Collaborate closely with GP practices and Senior Clinical Pharmacists.

• Participate in practice meetings, patient participation groups, and other forums to enhance the role of pharmacy technicians within the CPGP model and address issues pertinent to prescribing and medication optimization.

• Liaise with other pharmacy stakeholders, including ICB Pharmacy teams, community pharmacies, and hospital pharmacy teams.

• Support Medication Optimization and the execution of the Medication Optimization strategy.

• Manage function-specific information, which may be sensitive or complex, ensuring appropriate recording, transferring, and coordination in compliance with the Data Protection Act, Caldicott Guidelines, and the Confidentiality Code of Conduct.

• Organize, plan, and manage personal workload to meet established deadlines.

• Utilize SystmOne, the GP practice clinical system, to conduct prescribing audits.

• Engage in clinical audits and share outcomes to facilitate improvements and changes in practice.

• Act on major MHRA drug alerts as instructed by the ICB Pharmacist, advising on any medication changes that arise.

• Monitor prescribing expenditures at the individual practice level and suggest modifications to maintain budgetary constraints.

• Pursue learning and development opportunities to ensure the requisite knowledge and skills for practice-based work.

• Undertake additional duties that align with the skills and competencies of the post holder as service priorities evolve.

• Manage tasks related to COVID-19 vaccine management, including ordering vaccines and maintaining records as appropriate.

• Cultivate a culture that promotes equality and values diversity, adhering to the organization's Equality and Diversity policies and actively promoting relevant issues.

• Uphold principles of openness, transparency, and candor in all work practices.

The post holder will be expected to maintain constructive relationships with a wide range of internal and external stakeholders, including General Practice, the Clinical Pharmacy Team, and the Central London division of North West London ICB, while participating in relevant internal and external working groups/projects, services, and initiatives that develop the role and service within Central London.

Requirements

Registration: Mandatory registration with the General Pharmaceutical Council (GPhC).

Education / Training / Qualifications: GPhC-accredited course, GPhC-recognized qualifications, or a GPhC-approved apprenticeship pathway.

Knowledge and Experience: Recent experience in a comparable role within General Practice, previous experience in a patient-facing role, and familiarity with prescribing/clinical systems such as SystmOne.
