About the RoleThis is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to develop your skills and expertise as a healthcare professional. You will be responsible for consulting with patients, triaging referrals, and working collaboratively with other healthcare teams to deliver high-quality patient care.
Key Responsibilities- Consult with patients via telephone, video, or face-to-face, using EMIS to manage patient records and referrals.
- Triage referrals and calls from external clinicians, patients, and others, ensuring safe and effective patient care.
- Work collaboratively with other healthcare teams to deliver high-quality patient care, including emergency care and non-medical prescribing.
- Develop and implement effective management plans for patients, ensuring safe discharge, referral, and transfer to appropriate healthcare services.
- Participate in quality governance systems and processes, contributing to the improvement of healthcare services and patient outcomes.

- Experience as an Advanced Healthcare Practitioner or equivalent.

- Ability to prioritize tasks and manage workload effectively.
This is a permanent position, with flexible working hours and policies available. You will be based at one of our GP practices, with opportunities to work across multiple sites.
