About the Role

You will be responsible for consulting with patients, triaging referrals, and working collaboratively with other healthcare teams to deliver high-quality patient care.

Key Responsibilities
  • Consult with patients via telephone, video, or face-to-face, using EMIS to manage patient records and referrals.
  • Triage referrals and calls from external clinicians, patients, and others, ensuring safe and effective patient care.
  • Work collaboratively with other healthcare teams to deliver high-quality patient care, including emergency care and non-medical prescribing.
  • Develop and implement effective management plans for patients, ensuring safe discharge, referral, and transfer to appropriate healthcare services.
  • Participate in quality governance systems and processes, contributing to the improvement of healthcare services and patient outcomes.

  • Ability to prioritize tasks and manage workload effectively.
Working Arrangements

This is a permanent position, with flexible working hours and policies available. You will be based at one of our GP practices, with opportunities to work across multiple sites.
