This unique care establishment is housed in a beautifully converted Victorian residence, offering a warm and supportive atmosphere for our residents.
**Candidates must possess an NVQ/QCF Level 3 in Health & Social Care to be eligible for this position.**
As a Care Coordination Specialist, your primary responsibilities will include:
- Collaborating with a team to uphold the personal care of residents, ensuring their dignity is respected and independence is promoted.
- Guaranteeing that care services are delivered consistently at a high standard.
- Assisting in maintaining the residents' living environment and facilitating daily activities within the home.
- Taking charge of the home's operations and ensuring the wellbeing of both staff and residents in the absence of the home’s Manager.
- Managing your own shifts, administering and documenting medication, coordinating with healthcare professionals, and updating residents' computerized care plans.
- Supporting the induction, training, and oversight of support staff.
- Assisting residents with limited mobility or physical/learning challenges, utilizing mobility aids and personal equipment effectively.
The following qualifications and attributes are preferred for this role:
- Experience in supervising a care team.
- Empathy and understanding of the residents' needs.
- Strong leadership capabilities with effective communication skills.
- A positive, can-do attitude at all times.
The successful candidate for the Care Coordination Specialist position will enjoy a competitive hourly wage of £14.50, translating to an annual salary of £27,144.
