About the Role

We are seeking a highly motivated and empathetic individual to join our team as a Senior Health Care Support Worker. As a key member of our multidisciplinary team, you will play a vital role in supporting our patients through their reablement pathway.

Key Responsibilities

  • Work alongside the nursing team to support patients with their daily needs and promote independence.
  • Conduct observations, physical health checks, and documentation as required.
  • Support patients with their Activities of Daily Living (ADLs) and facilitate escorts off the ward as needed.
  • Participate in clinical supervision sessions and attend in-service training to meet individual and service needs.
  • Liaise with representatives from associated voluntary and statutory organizations to facilitate good communication processes.

Person Specification

  • Previous relevant experience working with people with mental health problems at a HCSW grade or equivalent within a health and social care setting.
  • Essential skills include physical healthcare interventions, IT skills, clear and accurate clinical note keeping, de-escalation skills, and planning and organization skills.
  • Desirable skills include immediate life support, venepuncture, and a Certificate in Community Mental Health.
