Role Overview: The successful candidate will join a dedicated care team focused on delivering community-based palliative nursing care. This role involves working closely with families of children and young individuals facing life-threatening and life-limiting conditions.
Key Responsibilities:
- Provide comprehensive holistic nursing care to children and young people with complex medical needs in their homes or other respite environments.
- Offer emotional support and guidance to children, young people, and their parent/carers, ensuring individualized care plans are developed, implemented, and evaluated during each visit.
- Collaborate with the end-of-life team leader to plan and deliver compassionate end-of-life care.
- Participate in an on-call rota, providing support during overnight and weekend shifts as needed.
- Deliver high-quality, individualized care that promotes empowerment and advocacy for children and their families.
- Supervise children and young people in the absence of their parents or carers, ensuring their safety and well-being.
- Communicate effectively with all members of the care team and other partner agencies.
- Adhere to clinical governance guidelines and promote evidence-based practices that enhance care quality.
- Manage your workload efficiently during respite visits and office duties.
- Ensure compliance with UHBW policies and Jessie May guidelines, maintaining health and safety standards.
