About the Role
We are seeking a dedicated and compassionate Caregiver/HCA to join our team at The Hospice of St Francis. As a key member of our community care team, you will play a vital role in providing high-quality care to patients in their own homes.
Key Responsibilities
- Deliver personalized care to patients in their own homes, ensuring their dignity, comfort, and well-being are maintained at all times.
- Work collaboratively with the care team to develop and implement individualized care plans that meet the unique needs of each patient.
- Provide emotional support and practical assistance to patients and their families, promoting independence and confidence.
- Conduct thorough assessments of patients' needs, identifying areas for improvement and implementing changes as necessary.
- Maintain accurate and detailed records of patient interactions, care plans, and outcomes.
- Participate in ongoing training and professional development to enhance skills and knowledge in areas such as end-of-life care, moving and handling, and safeguarding.
- Work flexibly to meet the needs of patients and the care team, including evenings, weekends, and bank holidays.
Requirements
- Care certificate/NVQ qualified level 2 or equivalent.
- 3 years' experience in a similar role, with a proven track record of delivering high-quality care in a community setting.
- Current driving license and use of own car.
- Knowledge of and experience in care at the end of life and after death.
- Current knowledge of Moving and Handling, Health and Safety, Safeguarding, and Mental Capacity Legislation and Guidance.
