Key Responsibilities- Provide 24/7 support to young people in their own homes, promoting their independence and well-being.
- Develop and implement individualized support plans to meet the unique needs of each young person.
- Assist with daily living tasks, such as cooking, cleaning, and personal care.
- Engage with young people in activities that promote their physical and emotional well-being.
- Communicate effectively with other professionals, including social workers and healthcare providers.
- Contribute to a safe and supportive living environment, adhering to all relevant policies and procedures.
- NVQ (QCF) Level 3 (or above) in Health & Social Care.
- Minimum of 12 months experience working with vulnerable young people.
- Excellent written and spoken English.
- A current DBS check.
- Full clean, UK driving licence.
- IT literacy - MS Office.
- Excellent communication, organization, and report writing skills.
- A flexible, empathic, and supportive attitude.
- The ability to work autonomously.
- Training in safeguarding, child protection, and risk assessments beneficial.
