Job Summary

An exciting opportunity has arisen for a highly motivated and experienced Specialist SEND Health Visitor (Band 7) to join our dedicated team at Central London Community Health Trust.

As a key member of the Health Visiting Team, you will play a pivotal role in leading and managing a quality improvement project focused on children with Special Educational Needs and Disabilities (SEND).

Main Responsibilities

  • Lead and manage a quality improvement project aimed at enhancing the support provided to children with SEND within our community.
  • Collaborate closely with multidisciplinary teams, including educators, therapists, and social workers, to develop and implement effective interventions.
  • Conduct comprehensive assessments of children's needs and develop individualized care plans.
  • Provide education and support to families on a range of SEND-related topics.
  • Monitor and evaluate the effectiveness of implemented interventions and make necessary adjustments.
  • Advocate for the rights and needs of children with SEND within the community.

Skills and Qualifications

  • Registered Nurse or Health Visitor with a specialization in child health.
  • Extensive experience working with children with SEND.
  • Strong leadership, communication, and interpersonal skills.
  • Proven ability to manage projects effectively.
  • Commitment to evidence-based practice and continuous professional development.
