As a key member of our Children's and Families Care Group, you will play a vital role in delivering the Healthy Child Programme to children, young people, and families across our region.
Key Responsibilities- Work closely with multidisciplinary teams and agencies to address a range of care and support issues across the spectrum of conditions.
- Contribute to the prevention of unnecessary hospital admissions and the improvement of the health and wellbeing of the population.
- Collaborate with Team Leaders and Professional Leads to ensure synergy across localities and deliver services to a high standard.
- Be responsible for a defined population of service users in various settings, including community and hospital environments.
- Assess children and young people's care and support needs, develop, implement, and evaluate care and support packages.
- Take on supervisory, training, and leadership responsibilities, with some responsibility for team performance and service developments.
- Work with the team leader to undertake delegated tasks in team management.
- Deliver commissioned aspects of the Healthy Child Programme.
