We are seeking an experienced Advanced Nurse Practitioner to join our Primary Care Network team at North East Derbyshire Primary Care Network Limited.

• Provide high-quality, patient-centered care to individuals and communities in a primary care setting.
• Work autonomously within your professional scope of practice to deliver face-to-face, telephone, and home visits.
• Operate within your sphere of competence and agreed acceptable limits of practice to provide expert clinical advice to patients, carers, and colleagues.
• Prioritize patient needs, making referrals for investigations as necessary.
• Collaborate with GPs and other healthcare professionals to deliver proactive, personalized, and integrated health and social care.
