Job Title: Family and Networks Practitioner

Steppingstones Medical Practice is seeking a highly skilled and dedicated Family and Networks Practitioner to join our team.

Job Summary

The successful candidate will be responsible for managing a busy and interesting clinical caseload, working as part of a team of practice GPs, nurses, and other healthcare professionals to deliver high-quality general practice services to patients at Steppingstones Medical Practice.

Key Responsibilities
  • Undertake a variety of duties including surgery consultations and emergencies, telephone consultations and queries, on-call, visiting patients at home, checking and signing repeat prescriptions, and dealing with clinical queries, patient test results, patient medical reports, and examinations.
  • Make professional decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers within the organization.
  • Supervise and support trainee doctors.
  • Comply with the requirements of the practice's General Medical Services (GMS) contract, Enhanced Services contracts, and educational/training delivery contracts.
Clinical Responsibilities
  • Provide high-quality medical services to patients registered with the practice.
  • Refer patients as appropriate and in accordance with local protocols and procedures.
  • Record clear and contemporaneous I.T.-based consultation notes to agreed standards.
  • Assess the healthcare needs of patients with undifferentiated and undiagnosed problems.
  • Screen patients for disease risk factors and early signs of illness.
  • Manage long-term conditions in accordance with practice, local, and national clinical guidelines.
Non-Clinical Responsibilities
  • Maintain clinical records (manual/electronic) in line with good practice and in accordance with practice procedures, including appropriate coding as agreed in the practice.
  • Maintain administrative records in relation to GMS contract, Enhanced Services contracts, and Long Term Conditions Framework and becoming LTC lead, including coding.
  • Work as part of a team to ensure targets are met.
  • Attend and participate on a regular basis on in-house clinical and educational meetings.
Requirements
  • Must have a strong understanding of general practice and primary care.
  • Must be able to work effectively as part of a team.
  • Must have excellent communication and interpersonal skills.
  • Must be able to maintain confidentiality and handle sensitive information.
