As a Lead Community Nurse, you will be responsible for providing exceptional care in a 16-bed Community Hospital. This role involves working with a diverse patient population, including those transitioning from acute care settings and individuals requiring ongoing rehabilitation. You will also support patients with palliative needs and those undergoing various day case procedures.
Key Responsibilities:
- Deliver high-quality, patient-centered care in collaboration with a multi-disciplinary team.

- Participate in the rehabilitation process for patients moving from acute care to community settings.
