About the Role:

As a Care Coordinator, you will be an integral part of our care team, working closely with our care staff and other professionals to ensure that our clients receive the highest level of care and support. You will be responsible for coordinating the delivery of care services, ensuring that all necessary arrangements are made and that our clients receive the care they need.
