Position Overview
The successful candidate will deliver expert psychiatric care to individuals engaged with the East Conwy Community Mental Health Team (CMHT). Key responsibilities encompass conducting specialized psychiatric evaluations, treatment, and management for adults experiencing significant mental health challenges. The consultant will work collaboratively with both primary and secondary healthcare teams to identify the most suitable care pathways for referred patients.
Moreover, adherence to the Mental Health (Wales) Measure 2010 is essential, and the consultant may provide guidance to junior medical staff as required. They will also contribute medical expertise to patients under the secondary care CMHT and oversee Primary Mental Health Services within their designated area.
The service operates within an Acute Care Model, and the consultant will engage with a multidisciplinary team to ensure effective clinical leadership. Regular collaborative meetings will be held to discuss patient care strategies and transition plans.
